  1. Log into Assurance

  2. In the top left-hand corner, click 'Registers & Forms

  3. Select 'Safety Incident Form' 

  4. A list of logged incidents will appear. Select the title to manage incident

  5. Here, you may view, escalate, export to PDF, or close and incident

  6. Here, you may also assign an action, check or upload attachments, of create a checklist / merge doc
