How to access the public link of your form

Step-By-Step Guide

  1. Select the Gearwheel in the top right hand corner to access the Administrator Hub

  2. Click Registers & Forms Templates

  3. Find and select your Form (eg Incident Form or Provider Due Diligence)

  4. Look for the 'public link' tab

  5. Press Copy Link



How to link a form to PolicyPlus site

Step-By-Step Guide

  1. Copy the Form Link (eg Safety Incident Form)

  2. Navigate to PolicyPlus site

  3. While in edit mode, select the content item you wish to link to the Form

  4. In the Link tab, select external link and Paste the Form Link

  5. Save and refresh, then publish changes
